1) Collect a paper form from the pro shop
2) Select your 4 players
3) Hand the completed form to a member of the pro shop team along with £10 for entry
4) Please return all paper entry forms by 5pm on Wednesday the 10th of April.
Online entries close by midnight Wednesday 9th April by midnight
